-      Original graduation fee payment receipt

-      Original return of graduation gown form

-      Duly filled certificate application form

Nil; however, payment of approved storage fees for certificates that are overdue is required.

 

 

-Certificates and transcripts will be issued as per the certificate issuance schedule
